CPD Activity Log

The PDH Activity Log is provided for your use and must be completed and submitted to the Board only if you are audited. You may make additional copies of this form if necessary. The detailed list of your activities showing categories as provided by the rule should be completed and totaled to ascertain the current PDH credit.

You should pay close attention to the instructions on the form.

The Activity Log is available in four file formats: Adobe Acrobat (PDF), Microsoft Word (DOC), Rich Text Format (RTF), WordPerfect (WPD), and Microsoft Excel (XLS).

Except for the Adobe Acrobat file, all file formats allow you to use your personal computer to fill in the data on the form. The Microsoft Excel spreadsheet divides the Activity Log into three "worksheets" named Instructions, Activity Log, and Notes. The sheet named Activity Log includes some cells with formulas which provide totals. This is probably the easiest file format to use.

The Download Package is a self-extracting, compressed (zip) archive file which you can save to your local disk. This includes the CPD Instructions, the Board Rules regarding CPD, a sample renewal invoice, and two versions of the CPD Activity Log, the PDF, and XLS file formats. (Detailed instructions about downloading this type of executable, compressed (zip) archive file can be found on the Application Forms page; however the file names will be different)

The default location for the Download Package on your local drive is C:\LA CPD.
