All PDF documents on this page can be filled out and saved to your hard drive using Adobe Reader.

Follow these steps for uploading completed applications (including experience records) directly into our system:

  1. When you have completed filling out the form(s)/application(s), save them to your Desktop.
  2. In the File Upload area below, click "Browse," then navigate to your Desktop where you saved your document(s).
  3. Click on the document in the pop-up window and then click "OK".
  4. Click "Upload" to send your document to us.
  5. Repeat steps 1-4 for additional documents.
  6. To pay your fees, click and print out the Remittance Form - DO NOT SEND ANOTHER APPLICATION 
  7. ***At this time, CPD documents will not be accepted via this upload function. Please continue mailing them to LAPELS.***
